These files are in Adobe Acrobat form.   You can download them to your Computer.  In Microsoft Explorer - right click on the link and save to file.  In Netscape do a shift key and left mouse button and save to file.

 

 

General Forms and Information

The BSA has designed a new Annual Health and Medical Record.  Existing Class 1, 2, and 3 forms can continue to be used in 2009, but the only supported form effective January 1, 2010, is the Annual Health and Medical Record. Its use will be mandatory for the 2010 National Scout Jamboree. The links below will take you to the two types of the record and a FAQ about it:
